We can't respect your choices for future medical care...unless we know what they are. Ask us about advance care planning.
Advance Care Planning is a process for you to:
Understand possible future health choices
Reflect on these choices in light of the values and goals important to you
Discuss your choices with those close to you and the health professionals who care for you
Make a plan for future healthcare situations
This free workshop will address your questions and help you complete an Advance Directive, which allows you to remain in charge of decisions about your medical care, even when you may not be able to speak for yourself.
